Scope
This Privacy Policy applies to information collected when you visit SpineLUX.com, contact us, or submit a website inquiry. Protected health information created or maintained while providing healthcare is governed separately by applicable law, including HIPAA when it applies, and by the relevant Notice of Privacy Practices.
Information we may collect
Depending on how you use the site, we may receive contact information you choose to provide, such as your name, telephone number, email address, preferred location, contact preference, law-firm name, and referral contact information. The website or hosting provider may also process basic technical information such as browser type, device type, pages visited, approximate location derived from an IP address, referring page, and error or security logs.
Do not submit diagnoses, medical records, dates of birth, claim numbers, government identifiers, or other sensitive medical or legal information through ordinary email or a general website form.
How information may be used
- Respond to appointment, contact, and attorney-referral inquiries.
- Coordinate an appropriate follow-up conversation.
- Operate, secure, troubleshoot, and improve the website.
- Comply with applicable legal, regulatory, and administrative obligations.
- Protect patients, visitors, SpineLUX, and others from misuse or security threats.
How information may be shared
Information may be shared with service providers that support website hosting, communications, security, analytics, or form delivery; with affiliated healthcare operations involved in responding to a request; or when required or permitted by law. Service providers are expected to process information only for authorized purposes and subject to appropriate contractual safeguards.
SpineLUX does not sell medical information submitted for purposes of receiving care. We do not use general website forms to authorize the release of medical records.
Cookies and analytics
SpineLUX uses Google Analytics 4 to understand how visitors find and use the website and to measure actions such as page views, consultation-request activity, telephone-link clicks, and secure attorney-referral link clicks. Google Analytics may process device and browser information, referring pages, pages visited, approximate location derived from an IP address, and cookie or similar identifiers.
SpineLUX configures the website measurement tag without Google advertising signals or ad-personalization signals. Names, telephone numbers, email addresses, form entries, medical records, diagnoses, and other sensitive information must not be sent to Google Analytics. Browser settings and available Google controls may allow you to limit analytics cookies, although doing so can affect measurement.
Retention and security
Website information is retained only as reasonably necessary for the purpose collected, operational requirements, and applicable legal obligations. No website or email system can be guaranteed completely secure. Sensitive records should be exchanged only through an approved secure process.
Third-party websites
This site may link to maps, social-media services, and other third-party websites. Their privacy practices are governed by their own policies, and SpineLUX is not responsible for their content or data practices.
Children’s privacy
This website is intended for general audiences and is not designed to collect information directly from children without involvement from a parent or legal guardian.
Your choices and questions
You may choose not to submit a website form and contact intake by telephone instead. Questions about website privacy may be directed to 972-920-9111 or intake@spinelux.com. Requests involving clinical records will be handled through the appropriate medical-records process.
